Our office will call 24 - 48 hours prior to your appointment to confirm.
For your convenience, you can now download and complete your New Patient Registration Forms before coming into the office.
Cancellations/ No-shows

$25 fee for no-shows/ cancellations within 24 hours of theh scheduled appointment

*Please bring your current medication list with you to EACH appointment.
When you are in need of a prescription refill, please contact your pharmacist and have them fax a refill request to your Clinical Renal office. Please leave room for a 48 hour turn around time for prescription refills.
